Track 12. Molecular Targeted Cancer Therapy and Precision Medicines

Targeted Cancer therapies are drugs or medication that prevent the growth and spread of cancer by interfering with molecular target that are involved. Targeted therapies also called as Molecularly targeted cancer therapies, Molecularly targeted drugs. Chemotherapy and Targeted therapies differs in several ways: the Targeted therapies act on the specific Molecular target and block tumor cell proliferation whereas Chemotherapy acts on rapidly dividing normal and Cancer cell and kill cell. Molecular targeted therapies are currently focusing on anticancer drug development and they are the cornerstone of precision Medicine to prevent, diagnose and treat diseases.

Precision medicine or personalized medicine is an approach to individual care for selecting treatment that are helpful to the patients based on a genetic understanding of their disease. Depending upon the type of cancer, its size and proliferation, combination of Treatment surgery, Chemotherapy, Radiation therapy and Immunotherapy may vary. But in precision medicine, information about genetic changes in the tumor can be helping to decide the Treatment.

Track 19. Cancer Epigenetics

Cancer Epigenetics is the study of Epigenetic modification of the genome of Cancer cell that do not change the DNA sequence. Epigenetics alteration are important for prevention, detection and treatment of cancer. There are two primary Epigenetic mechanism -DNA methylation and covalent modification of histones. The specific Epigenetic profiles as tool to diagnose individuals type of cancer are more specific and accurate. Different Epigenetic profile is used to determine the stage of development of a Cancer in a patient.

Track 20. Cancer Preventive Vaccine

Cancer vaccines stimulate the immune system’s ability to protect the body against the infectious agents that may cause disease. Cancer vaccine are also known as biological response modifiers. There are two types of cancer vaccine: Preventive vaccine and treatment vaccine. The Preventive vaccine is used to stop the growth of the Cancer cell and treatment vaccine are used to treat the existing cancer by natural immune system to fight against the cancer.

Cancer is a life-threatening disease across the world, almost 8.2 million cancer related death occurred and 14 million affected with new cancer cases. So, professionals from the health care are trying to reduce the rate of death due to cancer by detecting in the early stage of the cancer disease by promoting regular checkup. The early diagnosis of the cancer disease will increase the success percentage of the treatment.

Cancer Diagnosis

The market size of diagnostic cancer globally was valued at USD 124.0 billion in 2016 and it is expected to grow up to 7.2% in the upcoming period. The expected growth in cancer diagnostic market is due to advancement in diagnostic technique, demand for the oncological screening tests, screening tools, imaging modalities, awareness and support provided by the government in all over the world. The cancer research organization invested on the cancer research projects along with the government, public and private companies to launch an advanced technique in the diagnostic of the cancer in early stage.

Cancer diagnostic market in North America, Asia, Europe, Africa, middle east county and other country is expected to invest large amount in the diagnostic market. Asian market has highest growth in the CAGR and serve as a revenue pocket for the diagnostic companies. The cancer diagnostic market companies have advancement in the imaging technology by hybrid imaging instruments such as SPECT/CT and PET/CT.

The companies of the cancer diagnostic market are GE Healthcare (U.K.), Agilent Technologies, Inc. (U.S.), Thermo Fisher Scientific, Inc. (U.S.), Illumina, Inc. (Denmark), Becton, Dickinson and Company (U.S.), C.R. Bard, Inc. (U.S.), QIAGEN N.V.  (Netherlands), Abbott Laboratories, Inc. (U.S.), Roche Diagnostics (Switzerland), Siemens Healthcare (Germany), Philips Healthcare (U.K.), and among others.

Cancer Therapeutics

The cancer treatment/drug market is estimated to touch $111.9 billion by 2020, registering a growth of 7.1% from the period of 2014- 2020. Some of the therapies for the treatment of cancer are immunotherapy, chemotherapy, monoclonal antibodies. Among these monoclonal antibodies have high market value due to their efficacy in treatment.

The advancement in the treatment of the cancer has been brought in 2016. The FDA approved new eight cancer treatment and 12 new uses of approved cancer therapies. The new approvals include targeted treatments for hard-to-treat forms of lung and kidney cancers, chronic lymphocytic leukemia (CLL), and multiple myeloma, immunotherapies for bladder cancer and multiple myeloma. New-use approvals have broadened treatment options for patients with melanoma, sarcoma, CLL, lymphoma, neuroendocrine tumors, and breast, lung, kidney, and head and neck cancers. In addition, the FDA approved the first liquid biopsy test in 2016.

Targeted therapy report for different types of cancer

Acute myeloid leukemia(AML) is the most common type of cancer detected in human beings. In US almost 19950 people had AML with this only 27% of the people survive after cancer detected. Researches in 2015 reported new therapy for AML with mutation in a gene called FLT3. Patients with FMS-related tyrosine kinase 3 (FLT3)–positive AML have a reduced prognosis and a high chance of relapse. In clinical trial the patient lives year longer after therapy of both FLT3 and chemotherapy when compared to patient received with chemotherapy alone.

Hormone receptor–positive breast cancer is the most common type of breast cancer.  Treatment resistance and significant challenges are faced in hormone(antiestrogen) Therapy, although it is a standard care for patient with breast cancer.  In clincial trial, ribociclib noticeably slowed cancer progression. After 18 months, cancer had not worsened in 63% of patients in the ribociclib group, when compared with 42% of patients in the placebo group.

Renal cell carcinoma (RCC) is the common type of kidney cancer in adults.  Cabozantinib is an oral treatment that blocks targets in cancer cells, including tyrosine kinases MET, VEGFR2, and AXL. Severe-grade adverse effects that occurred more frequently with cabozantinib than with everolimus included high blood pressure, diarrhea, and fatigue. Only 40% of patients are relapse with metastasis after stage III RCC kidney cancer surgery.

In Ovarian cancer clinical trial, 4 of 10 women with folate receptor alpha–positive ovarian cancer resistant to standard platinum chemotherapy sensed tumor shrinkage after receiving IMGN853 (mirvetuximab soravtansine).

Neuroblastoma is the soild tumour most common in children. Almost 700 children are affected with neuroblastoma in US. Neuroblastoma requires chemotherapy, radiation therapy, and/or autologous stem cell transplant (ASCT), which may also include surgery. Despite treatments, fewer than half of children will survive 5 years after diagnosis of a neuroblastoma.

Pancreatic cancer is often difficult to diagnose. As a result, it is often not found until it has spread from the pancreas to other parts of the body and can no longer be removed with surgery. Overall, only 29% of patients with pancreatic cancer will be alive 1 year after diagnosis. As the fourth leading cause of death resulting from cancer in the United States, pancreatic cancer took an estimated 42,000 lives in 2016.

The patient with colorectal cancer live longer if the cancer is detected in early stage on the left side of the colon than on the right side. patients received a combination of FOLFOX (fluorouracil, leucovorin, and oxaliplatin) or FOLFIRI (fluorouracil, leucovorin, and irinotecan) chemotherapy with either of the two standard targeted therapies for colorectal cancer. The rates of surgical success were found to be substantially lower among patients with rectal cancer who underwent laparoscopic surgery than among those who underwent open surgery (82% v. 87%).
